Dear Student,
You are receiving this email because Brown has not received confirmation of your vaccination status, which has implications for your enrollment. As shared in multiple communications since our July 7 email on COVID-19 vaccine guidance, Brown is requiring all students to receive their final dose of the COVID-19 vaccine and upload their vaccination card by Aug. 1, 2021 in order to achieve near-universal vaccination before fall semester activities begin. This will enable a more traditional fall semester with more in-person activities.
By August 1, this means that every student who will be on campus for the Fall 2021 semester must either:
- Be fully vaccinated and have uploaded documentation of their final vaccine dose to the Health & Wellness Patient Portal; or
- Complete this survey notifying Brown of the date in August on which they will be fully vaccinated.
You have not yet uploaded your full vaccination record or completed the survey. We encourage you to verify your vaccination status as soon as possible to ensure that you meet this important deadline. If you are not in compliance by August 1, your enrollment may be cancelled for the fall semester, and you will not be permitted to register for classes or access any campus facilities.
The only exceptions for compliance will be for students who have a medical or religious exemption from vaccination that has been approved by University Health Services and international students who have made arrangements to be vaccinated in Rhode Island via the survey above because they do not have access to vaccines in their home countries.
How to get a vaccine:
Please refer to the Healthy Brown Vaccination page for further instruction on where to find a vaccination site near you. All students should follow the local, state, and national guidelines to register for and receive a COVID-19 vaccine approved by the FDA-EUA or WHO-EUL. U.S. residents can find vaccine availability on Vaccines.gov.
How to upload your vaccination information:
Students should upload their vaccination cards directly to the Health & Wellness Patient Portal as soon as they receive a vaccine dose, even if it is the first of a two-dose regimen. We recommend you access the patient portal via desktop as it will allow for an easier upload than mobile. Detailed instructions are available through the Verify Your Vaccination page on the Healthy Brown website.

If you are participating in a clinical trial studying an approved FDA/EUA or WHO/EUL vaccine, please provide documentation from the Study Coordinator confirming your participation and the "arm" of the study to which you are or were assigned. Please email documentation to nursing@health.brown.edu.
