Following President Paxson’s announcement that all employees will be required to receive the final dose of the COVID-19 vaccine by July 1, 2021, University Human Resources (UHR) is providing the community with information on the process for requesting a medical or religious exemption.

You must also download the medical exemption form to be completed by the employee and employee's medical provider (a licensed physician, physician’s assistant or nurse practitioner) here: https://brown.edu/go/vaccinemedicalexemption

Requests for medical exemptions will be reviewed by a panel of physicians. Requests for religious exemptions will be reviewed by UHR in consultation, as necessary, with the Office of Institutional Equity and Diversity.
