Employee Acknowledgement

Faculty, staff, and student workers who work at a Brown location or are working remotely are required to complete COVID-19 Campus Safety training, which includes acknowledging their agreement to comply with health, prevention and safety protocols.

For each employee or student worker who will work at a Brown location or remotely, an acknowledgement form will be included in a training module in Workday upon completing COVID-19 Campus Safety Policy training. For employees with limited access to Workday, including student workers, University Human Resources will directly provide access to COVID-19 Campus Safety training.

Upon completing the training, employees will need to agree to each element of the Employee Acknowledgement.

Brown University Acknowledgement of COVID-19 Campus Safety Policy

  1. I acknowledge that I have read the Brown University COVID-19 Campus Safety Policy (the “Policy”). I agree to follow this Policy at all times while working onsite, and if working in a non-Brown location, I agree to follow the rules or guidelines set by that location or the Principal Investigator. If I have any questions about this Policy or about my responsibilities, I will ask my supervisor or a member of University Human Resources (“UHR”).
  2. I understand that Brown will provide me with several reusable cloth face coverings or masks. I agree to wear a face covering or mask (University-provided or self-provided) in accordance with the Policy. I acknowledge that accommodations will be made for individuals with health conditions preventing them from wearing a face covering.
  3. While the 2019 Novel Coronavirus (COVID-19) emergency continues, I acknowledge that as a condition of my employment and/or my ability to perform research for, at, or under the auspices of Brown, I may be asked to undergo periodic testing for COVID-19.
  4. I certify that I currently do not have any symptoms known to be associated with COVID-19 as established by the U.S. Centers for Disease Control and Prevention (CDC) and stated in the Policy. If I begin to exhibit any of these symptoms, I agree that I will immediately notify UHR and leave my work location, unless I am working remotely, and that I shall not report to my Brown workplace until approved to do so.
  5. I agree that if I test positive for COVID-19, I must immediately notify UHR and leave the workplace, unless I am working remotely, and I will not report to my Brown workplace until I provide medical documentation clearing me to return. If I have tested positive prior to today’s date, I agree not to report to my Brown workplace unless I have been cleared by my health care provider.
  6. In addition to wearing a face covering as directed by the Policy, I agree to use my best efforts to prevent the spread of COVID-19, including maintaining a physical distance of at least (6) feet from others, washing my hands often or using hand sanitizer when hand washing is not immediately possible, and covering my mouth and nose when coughing and sneezing.
  7. I agree that if I do not know how to safely use equipment, tools or cleaning/disinfecting products that are required for my job or my research, I will contact my supervisor.
  8. If I believe that required workplace safety policies and practices are not being followed, I will immediately raise such issues with my supervisor or a member of UHR, or use Brown’s Anonymous Reporting Hotline.
  9. I recognize that no Brown employee, student worker, or researcher who makes a good faith report of suspected improper conduct shall be subject to retaliation, including harassment or any adverse employment consequence, as a result of making a report.
  10. I acknowledge that the Policy is subject to modification as new information and guidance becomes available, and that Brown requires every employee, student worker, and Brown researcher to comply with all workplace safety policies and practices. I understand that a failure to do so may lead to disciplinary action up to and including termination of my status at the University.
